From our experience

Half-letter (5.5"×8.5") is by far the most popular program size, compact and easy to hold. 8 pages is a simple bulletin; 12–16 pages adds song lyrics, sermon notes, and announcements. Order 10–15% more than your typical attendance to cover walk-ins and visitors.
